The Town Clerk’s Office is an official acceptance facility for the purpose of processing new passport applications and providing renewal application forms.

NATIONAL PASSPORT SERVICES HAS ADVISED LOCAL ACCEPTANCE FACILITIES THAT PROCESSING TIMES FOR PASSPORTS HAS RETURNED TO NORMAL TIME FRAMES
ROUTINE PROCESSING IS NOW 4 - 6 WEEKS
EXPEDITED PROCESSING MUST BE USED FOR TRAVEL WITHIN 2 - 4 WEEKS

NEW PASSPORT FORMS & FEES HAVE BEEN IMPLEMENTED Old forms can not be accepted.
New regulations require children 16 & 17 must appear with one or both parents.
A Passport Card is now available for LAND & SEA travel to Canada, Mexico, Bermuda and the Carribbean. The Card may NOT be used for travel by AIR.

Passport Application Requirements:
» DS-11 Passport Application Form
o This form should be completed in advance -- no white-outs, cross-outs or erasures are permitted
o DO NOT SIGN THE APPLICATION ahead of time
o An Oath must be taken by the applicant at the time of signing the application
» Proof of Citizenship (one of the following)
o Long, certified birth certificate
o Expired passport
o Naturalization Certificate
» Proof of Identity
o Valid driver’s license
o Military ID or other government-issued identification
o Naturalization Certificate (if photo likeness is good)
» Two identical 2” x 2” current passport photos
» Proper Fees
o Passport Fee -- check only payable to “U.S. Department of State” (separate checks each application)
o Execution Fee -- cash or check payable to “Town of North Reading”
o Expedite Fee -- persons traveling within 2 - 4 weeks must add “expedite” fee to Passport Fee
ALL DOCUMENTATION MUST BE SUBMITTED WITH THE APPLICATION AND WILL BE RETURNED

APPLICATIONS REQUIRE MINIMUM OF 4 - 6 WEEKS FOR PROCESSING Persons traveling within 2 - 4 weeks must expedite application Persons traveling within 2 weeks must call 1-877-487-2778 to schedule an appointment in Boston. Passports are valid for 10 years (age 16 and older); and for 5 years (under age 16).
ALL APPLICANTS MUST APPEAR IN PERSON REGARDLESS OF AGE All persons 16 years and older must subscribe to an Oath & sign application. Children 16 and 17 years old must appear w/one or both parents/guardians & all must sign. Children under 16 must appear with both parents & parents will sign application. [If either parent unavailable, a notarized consent from absent parent, evidence of sole custody, death certificate, or notarized explanation why absent parent’s consent cannot be obtained is required.] |
DS-82 – “RENEWAL” of a PASSPORT
If you: (1) can submit your current passport; (2) it was issued within 15 years; (3) you were at least 16 years old when it was issued; and (4) it was issued in the same name (or your name change was through marriage or court order & you can submit proper documentation), you may apply directly for a renewal. Processing fees apply as above; no execution fee.
Without further notice, the Town Clerk’s Office reserves the right to limit or suspend processing applications during Elections and Town Meetings or as the need arises to meet statutory obligations & deadlines |
